Due to the confirmed COVID-19 cases of the Dunmu Naval Fleet announced by CDC on April 19, 2020, all the faculty members, staffs and students need to follow the instruction below to report your status:

1. Anyone who is the relatives of the confirmed cases must fill up the attached questionnaire file immediately.

2. Anyone (such as a girlfriend or boyfriend) who has close contact with the confirmed cases lately must fill up the attached file immediately.

3. Anyone who happened to visit the seven hotspots at the specific time reported by the Kaohsiung City Government should implement 14 days of autonomous health management (Wear masks, avoid taking public transportation, and maintain social distance). Please also complete the attached questionnaire file.

4. Anyone who has received notification from the Central Epidemiology Control Center please fill up the attached questionnaire file and implement 14 days of autonomous health management.

5. Further instruction will be given to the one who belongs to the first and second categories.
